Start date – guide to completing the necessary forms

Welcome to the guide to filling out the necessary forms via the link you received in your email from the HR department representative. You also received a pre-filled document in the attachment to your email, categorized according to the type of work:

application for a medical fitness assessment, including an invitation to our contractual partner (risk group II)

Before you start work, it is necessary to complete the necessary formalities and provide us with the information listed below so that we can prepare the necessary documents (employment contract, wage statement) in sufficient time and forward them to the relevant department at UJEP where you will be working.
Your cooperation will help us facilitate the recruitment process and ensure that you can start work on the expected date. Please note that without an initial medical examination, we cannot process your employment contract and you cannot start work.

Personal questionnaire

At the following link Personal Questionnaire, you can download a personal questionnaire form, which you should fill out carefully, save in the usual way, and send as soon as possible to personalni@ujep.cz. In the subject line of the email, please include your start date and your first and last name. Please also pay attention to the education section and fill in your fields of study, including the date you completed your education.
If you are unsure, the necessary information will be completed during a personal meeting with a human resources representative.

Occupational medical examination – initial

Before starting work (category II occupational risk), you must undergo an initial medical examination with our contractual partner on the date specified in the invitation. Please follow the instructions provided in the invitation.

Personal visit

After completing the initial medical examination (before the start date), please come to the UJEP Human Resources Department (brown building of the Rector’s Office, 1st floor, office number 1.10). Office hours Mon – Thu 8:00 a.m. – 3:00 p.m. (11:00 a.m. – 11:30 a.m.) lunch break

Please bring the following documents with you:

– health insurance card
– credit sheet

– confirmation of insurance contributions from minimum wage (part-time employment)

During your personal visit, you will be informed about the next steps – a visit to the payroll department, where you will receive important information about occupational health and safety and a handbook for new employees.
