Application for a contribution from the social fund
Please fill out the application and submit it to the HR department. If you are applying for a contribution to a supplementary pension insurance plan, please provide a copy of the contract.
This document should include the employer’s consent to the contribution, including the account number to which the employer can regularly send the amount.
